WILMINGTON, DELAWARE (12/16/2014) A federal grant of $35 million to improve health care and cut costs has been awarded to Delaware, state officials say.
The four-year grant was announced by the Center for Medicare and Medicaid Innovation.
According to a statement from Gov. Jack Markell’s office, the state’s Health Care Innovation Plan is intended to transform health care delivery from a fee-for-service system toward one that rewards quality care and efficiency.
The grant will support primary care practices, population health initiatives, health information technology, workforce training and development as well as clinical and payment innovation.
